Closing Letter

I am writing to advise you that I will be closing my practice, All Star Pediatrics, and will no longer be available to provide your child’s/children’s medical care effective 12/1/2021. I will be available until that time for your child’s health care needs.

Please select another physician within that time frame to continue your care. Please consult with your insurance company to make sure that your new provider is in network with your plan. As all insurance plans are complex and varied and it is best to consult with them before choosing a new provider to ensure that your visits will be covered.

Your child’s medical records are confidential, and a copy can be transferred to another doctor or released to the child’s parent or guardian. Please determine your new medical home and sign a release of records as soon as possible so we may transfer your records to your new doctor. Please contact Privia Customer Service at 888-774-8428 to facilitate record transfer. Those of you who have signed up with MyPrivia Portal should have access to your records through the portal for the next 120 days. We can always provide a paper copy, but this is a cumbersome process and cannot be accomplished the same day it is requested. Please be advised that records cannot be emailed. Secure print, portal or fax are the only approved methods of record transfer.

You may contact the office if you have questions, but please try the above mentioned methods first as we only have two staff members and we will be easily overwhelmed.

Do not contact the after hours service for information about your medical records. They do not have access to your records and will not release them to you.

I have enjoyed taking care of your children and forming relationships with your family. You will be greatly missed. Unfortunately, the multifaceted difficulties of an unprecedented pandemic coupled with starting a new business proved too much to bear.
